Therapy Dogs United Prerequisite Checklist Questionnaire

As part of enrollment in our Therapy Dog Team Training Class and the Therapy Dogs United certification process, all handlers are required to complete this pre-screening form.

This questionnaire helps ensure that each handler and dog team is ready for therapy dog training. Answering “Yes” to any question does not automatically disqualify you or your dog. If needed, your responses may be reviewed by Therapy Dogs United to help determine the best next steps.
